The South West Self-Management Program:
The South West Self Management Program [SWSMP] is offered through the South West Local Health Integration Network. The goal of the program is to have a coordinated approach to support clients, caregivers and health care providers with self management.
Chronic Disease and Caregiver Self-Management Programs are FREE, highly interactive workshops that help you gain the confidence to manage your health conditions. Participants learn skills and strategies to make decisions around their health (physical, social and emotional health) working in partnership with all members of their health care team. We offer ongoing support after workshops to help you stay on track!

Fibromyalgia Support Network, in partnership with SWSMP, has been offering the LIVING A HEALTH LIFE WITH CHRONIC PAIN, a six-week workshop series, over the past few years. Several of our peer leaders have been trained to facilitate these virtual sessions. Join us for this interactive 6-week education series that will focus on:
- better breathing, visualization, gentle stretching, meditation
- improved communication with health care providers to maximize your appointments
- better sleep, medication and symptom management
- tips to reduce inflammation
- strategies to manage fatigue, breakthrough and ongoing pain
- decision making, problem solving and so much more.
Through the practice of skills and tools, you will learn how to better care for yourself and your condition. If you are living with chronic pain or are caring for someone living with chronic pain, seeking proven ways to make change, this workshop series is for you!
This is a Virtual Workshop: The workshop will be held live online. Each weekly session will be delivered using Zoom which will bring together workshop participants and leaders for interactive learning sessions. Priority will be given to those with a diagnosis of Fibromyalgia.
What you Will Need: Access to a computer, iPad, iPhone or android device with a webcam/video and audio in order to participate.

Dr. Eleanor Stein:
An influential ally of the wider fibromyalgia community in Canada and USA, Dr. Eleanor Stein is also a person who, since 1989, has been living with several chronic illnesses – Myalgic Encephalomyelitis, Chronic Fatigue Syndrome (ME, CFS), fibromyalgia (FM), and environmental sensitivities (ES). She has dedicated her professional life’s work to helping those also afflicted with these invisible illnesses, and advocating for change on multiple levels. Dr. Stein offers several virtual workshops and online courses. Most recently, she has started a monthly podcast, Live! with Dr. Stein! It is a subscription-based membership which offers 75-minute live sessions every other Thursday at 11 am MT and free access to its monthly Expert Speaker Series.
